2010/2011 Student-Athlete Medical Forms

 

New Student-Athletes (Freshmen & Transfers)

ALL new student-athletes must complete the Puget Sound New Student-Athlete Medical Forms packet and return them to the address below by August 1, 2010.  The front page of the document is a check list that you can follow to assure that you complete all documentation required for participation in intercollegiate athletics at the University of Puget Sound.  Delayed completion of these requirements may delay your ability to try-out or participate in any team activities. 

ALL new student-athletes must complete a pre-participation physical exam (PPE) using the University of Puget Sound Pre-Participation Physical Exam Form (page 2 of packet) to be completed by a physician within 6 months of the first date of participation in the intercollgiate sport (NCAA Bylaw 17.1.5).  The physician must sign and include a stamp from his/her office.

 

Returning Student-Athletes  

ALL returning student-athletes must complete the Puget Sound Returning Student-Athlete Medical Forms packet and return them to the address below by August 1, 2010.  The front page of the document is a check list that you can follow to assure that you complete all documentation required for participation in intercollegiate athletics at the University of Puget Sound.  Delayed completion of these requirements may delay your ability to try-out or participate in any team activities.

Tyler Vlasak
Football

Excitement continues to build as the fall season inches closer. The Logger football team will report in just a few days on Aug. 14. Tyler Vlasak will report as a team captain and one of the top Logger veterans. Vlasak was named a preseason All-American in June by the Concensus Draft Services after earning a spot on All-NWC second team in 2009. “It is rewarding to have a player of Tyler’s athletic ability also be a tremendous campus leader while maintaining a 3.0 GPA,” said head coach Jeff Thomas. “He is the epitome of a Logger Football player and we hope to duplicate his work into several others this year.”
